Personal Information

Personal information, such as name, age, telephone number, email, zip code, and similar information, is only collected on a voluntary basis. The user can, if desired, provide their age and answers to a short survey. If the survey indicates the user may meet the initial criteria for the study, the user can, on a voluntary basis, enter additional personal information (name and contact information) into a data form. There is no obligation to provide this information, even if you meet the initial study criteria identified in the “Am I a Candidate?” survey.

Acorn will only use personal information, such as name and contact information, in order to contact you to further discuss potential eligibility in the clinical study and for potential analytical/statistical purposes, or to improve the content and functionality of the website. Acorn will not disclose personal information to anyone who is not involved in the operation of this clinical study. Persons involved in the operation of the clinical study include Acorn employees and contracted employees, and study coordinators and physicians participating in the study. Acorn will not distribute or sell personal information to affiliates or third parties unless compelled to do so by law by a court order, subpoena, search warrant, legal process or regulation.

Anonymous Information/Usage Data

Additional anonymous information may also be collected to help Acorn understand, for example, the number of users visiting the website, the specific pages visited, the date and time the website was viewed and the domain name from which you access the internet. This anonymous information cannot identify you or be tied to you. It will only help Acorn gather information about viewing trends. Acorn may share anonymous information, such as cumulative statistics, with board members, contracted employees, and third parties at Acorn’s discretion.
